Enrollment instructions For groups of 3+ (employer-funded or voluntary): (1) complete and sign the Group Activation & Payment Authorization Form; (2) complete the Census Template (any format is fine as long as it has the required data). Your Financial Game Plan representative submits both to AllyHealth to activate coverage — Sam can help walk you through it.

Census, per member: first & last name, email, phone, date of birth, gender, address, plan name, and new/update/cancel status.
